I represent and warrant that I hold all licenses required by New York for every service I perform at NYSF, and that they are current and valid. I carry professional liability insurance of at least $300,000 per occurrence, naming NYSF Sunset Yards LLC, 341-353 39th Street LLC, and any owner or mortgagee of the building as additional insureds. I'll keep my license and insurance in force and notify NYSF within 24 hours if either lapses. I have read and agree to the Flex Member License Agreement, including its indemnification, hold-harmless, release, and conditions-of-access terms, and I authorize NYSF to charge my card on file $39 on the 1st of each month (plus $25/month if I add a locker), starting after my free first month, until I cancel. I consent to NYSF securely storing my ID and credentials to verify my identity, issue my building badge, and confirm compliance. See our Privacy Policy. Sign by typing your full legal name *
